Liberty Care Flintshire Ltd. provides support for individuals over the age of 18. We support individuals with learning difficulties, physical disabilities, sensory loss/impairment and provide specialist care for individuals with dementia and palliative care needs. Liberty Care Flintshire also provides care to help people whom have had strokes, amputees, wheelchair users, and people with heart conditions. We can assist with their care by manual handling individuals, and we can assist with medication to all individuals if required.

We recognise that individual’s needs will change. Our services are flexible in responding to these changing needs by altering the care plan and the amount of the care provision required from the commencement of the package to the end of life care as required. Our team of staff receive full and comprehensive training so are well equipped to deliver care to many different individual’s needs.

Liberty Care Flintshire Ltd. provide long term packages of care, as well as respite care while carers take a break. We also provide a reablement service and help the individuals to get back to their independent selves when possible which will result in no more care and independent living once again. We also provide a detailed and specific person-centred care plan, designed with the individual to ensure their needs, preferences and wishes are being met.

How the Service is Provided

Before we provide our services, we will ensure the needs and preferences of the individual are thoroughly assessed, and we can provide the care required. We will ensure that needs are assessed frequently and the care we provide has the flexibility to respond to the changing needs or requirements. The monitoring of care plans is on-going with senior staff members of the team regularly reviewing them. We arrange a meeting with the individual to establish the individuals most realistic and desirable aims and put together a person-centred care plan taking this into account, to support them to achieve their best possible outcomes. For example, a reablement approach to care for somebody returning home from hospital with the aim of regaining total independence.

The service we provide will support people to:

Be as physically, mentally, and emotionally healthy as possible by following the care plan and building a positive relationship with the individual. We will seek input from others when needed, i.e. GPs, OTs, DNs, and keep the next of kin informed as much as possible, all with the individual’s consent.

  • Be safe by completing risk assessments and ensuring staff adhere to them, seeking guidance from other professionals i.e. OTs, following health and safety legislation and by training staff.

  • Be involved in activities, hobbies or individual interests by encouraging service users to go out into the community and making them aware of what is going on in the area that may be of interest to them. We do this by providing a monthly newsletter to each individual we support, detailing activities in the area local to them.

  • Access education, learning and development opportunities by also providing details of any upcoming opportunities within the local area in the newsletter.

  • Have control over everyday life and where relevant participation in work by making a person-centred care plan with the individual and by respecting the needs and preferences of the individual. The person-centred care plan can be used as a tool to match carers to individuals with similar interests and so this will also help. We abide by all equal opportunities legislation and this further contributes to support individuals to maintain their won unique identities.

  • Maintain family and personal relationships; and devlop their potential , learn and practice life skills by setting realistic and achievable aims. We can use approaches to care, whereby small gains and achievements can lead to a bigger sense of achievement and purpose and by building and potential aims and desires to learn into the care plan, we can help the individual to achieve these aims, further enhancing their care experience and own sense of inclusion and self-value and own worth. We work alongside family and other personal relations involved to help the individual using our service to practice and develop their skills and independance as far as possible. We can offer advice on assistance we feel would be beneficial.

Liberty Care Flintshire Ltd. establish the preferred communication method and language of the individual we are providing to, prior to service commencement. We do not assume the preferred language of any individual and will seek to ensure that everyone is treated with dignity and respect, and this is line with the welsh government active offer initiative. We currently have welsh speaking members of staff to match to welsh speaking individuals. We also have a number of staff who would like to learn welsh and are on a waiting list with Flintshire County Council to attend a course, to further add to the numbers of welsh speaking staff. All staff are trained in communication and sensory loss and this further assists with supporting individuals with their communicaiton needs.
